At the time of appointment for both LSV and Sickness Benefit you must bring ALL forms that Work and Income have asked to be completed, failure to do so will result in the Nurse/Doctor not being able to complete the medical and you will be required to book a new appointment.
What you need to know about LSV (Limited service Volunteers)

One LSV appointment is booked per clinic as they are 40 minutes duration, this is a full medical examination all aspects, of your health will be discussed.

There are limited LSV appointments so there may be a wait until there is an appointment available. Once your appointment is made VIBE can provide you with a letter to Work and Income with the date and time of your appointment.

In order to make your LSV application smoother it is appreciated if you can contact your doctor and find out the date of your last Tetanus injection.

If there are no appointments available at VIBE there is always the option of seeing your own General Practitioner (GP), the cost this appointment can generally by Work and Income. But you will need to check with them first.
What you need to know about Sickness Benefit Certificates

A Sickness Benefit Certificate can only be completed by a General Practitioner (GP) Doctor or a Nurse Practitioner.

In order to avoid your benefit being suspended you need to ensure you book an appointment at least two weeks before your certificate needs to be renewed as we can’t ensure there will be an appointment available to you.

Every time you Sickness Benefit is renewed you will need a 40 minute appointment in order for the Nurse Practitioner/Doctor to assess your situation.

You will need to consistently see the same Doctor/Nurse Practitioner who completed your Sickness Benefit originally.
